CHAPTER ONE

ASSESSMENT OF PATIENT AND FAMILY

Assessment, according to Bailliere’s Nurses dictionary is defined as critical analysis and valuation of the status or quality of a particular condition, situation or other subject of appraisal. It is the systematic gathering of relevant information about health status of a patient and family, environment in which information is analyzed to make critical judgment. Assessment being the first phase of nursing process makes use of available tools and means of data collection, including observation of the patient, interviewing of the patient, relatives, friends and other health workers. Its effectiveness determines and affects the establishment of a good nursing diagnosis so as to render an efficient care to the patient. This chapter discusses into details the patient’s particulars, lifestyle and hobbies, and developmental history of the patient. The socio-economic status, past and present medical history, patients’ concept of illness, literature review on disease condition and validation of data.

Patient Particulars

Mr. G. A. A. is an 8 years old boy born on the 11th September, 2012 in Agogo Presbyterian Hospital in the Ashanti region of Ghana to Mr. Y. A and Madam C.A. He currently lives with his parents at Agogo. According to his mother, he schools at liberty Academy at jussi School and he is in class 2. He speaks Twi and English.  Mr. G. A. A is dark in complexion, about 60cm tall and weighs 24kg. He is the second child of his parents. He was born into Christianity and so is his elder sister and family members. He attends the Methodist Church with his family. Miss C. A  his mother, is his next of kin.
